Palliative care focuses on improving quality of life for patients and their families. Our team works alongside your doctor or specialist to coordinate care and manage symptoms. Palliative care is an extra layer of support that helps a patient understand and navigate their healthcare journey with confidence.
Our palliative care team can meet the patient at their home or wherever they call home. For those who prefer, visits can be provided in a clinic setting by appointment.
Who Can Benefit from Palliative Care?
Anyone living with a chronic, life-limiting, or life-threatening illness who is experiencing pain or other symptoms may benefit from palliative care. Symptoms may come from the illness itself or from treatment side effects. Palliative care also supports patients and families as they manage the challenges of a complex medical condition.
Palliative care is appropriate at any age and at any stage of a serious illness. Care can be provided together with curative or aggressive treatment, such as chemotherapy.
Basic eligibility requirements:
- Patient has been diagnosed with a serious illness
- Patient is seeking relief from symptoms related to the serious illness
- Patient is seeking additional support to improve quality of life and manage care
My husband has Parkinson’s, and it became difficult to get him in and out of the car to go to appointments. With palliative care, he could be regularly seen in our home by a Nurse Practitioner. The comfort & convenience of the in-home visits was great for both of us! Additionally, we knew that the palliative care team kept his regular doctor updated after each visit.

Medicare, Medicaid, and most insurance plans cover palliative care. As a community-based, 501(c)(3) nonprofit organization, we never turn anyone away based on their ability to pay.
We provide the same level of care for every patient, regardless of their ability to pay.

Palliative Care Team
Each patient has a dedicated palliative care team that works closely with their doctor or specialist.
This team consists of the following:
- Palliative Care Provider (Physician or Nurse Practitioner): Works alongside the patient's primary care and specialist physicians to manage symptoms, improve comfort, and coordinate treatments, medications, and equipment as needed.
- Medical Social Worker: Provides emotional support, helps with advance care planning, and connects patients and families with community resources.
- Registered Nurse (RN): Assesses patient needs, provides education and support, and conducts home visits when needed.
- Certified Medical Assistant: Coordinates appointments, assists with prescriptions, and supports communication between patients and the care team.
- Chaplain: Offers spiritual and emotional support that respects each patient's beliefs, values, and goals.
Patients receive regular home visits from members of the team, along with ongoing check-ins between visits to assess needs, answer questions and provide support.
Where is Care Provided?
- The patient’s home: This can be helpful if it is difficult for a patient to get out of their home.
- Long-term & Assisted living facilities: This allows an added layer of support for the patient and facility care team. Our palliative care team can meet a patient and family at the living facility.
- Hospitals: Our palliative care team can meet a patient and family right in the hospital. This can also allow our team to help with plans for a patient after discharge.
- Clinic: For patients who prefer to meet in a clinical setting, appointments can be made at our Statesville, Mooresville, or Taylorsville locations as needed.

It is important to get external support when you are caring for a loved one. Hospice & Palliative Care of Iredell County provides the following services for caregivers of palliative care patients:
- 24/7 Support: Our palliative care team is available day or night for questions about symptoms, medications, or what to do next.
- Nursing Support and Education: Our nurses help caregivers understand the illness, what to expect, and how to respond to changes, offering peace of mind during difficult moments. They also coordinate care among the care team, physicians, and facilities.
- Volunteer Support: Volunteers may visit patients in their homes, assisted living facilities, and nursing homes to provide companionship and emotional support. This can give the caregiver a break for a short time.
- Emotional Support: Our social workers, chaplains, and staff help caregivers cope with stress and uncertainty, and support families through grief before and after a loss.
- Grief Support: Continued grief support for families after the death of a loved one.
