While a physician's order is required to admit a patient to hospice or palliative care, anyone, including friends or family, can make a referral. If someone you know lives with a life-limiting illness and might benefit from our services, we welcome your call.
To begin the referral process, whether the patient needs in-home care, care in an assisted living or nursing home facility or inpatient care at the Gordon Hospice House in Statesville or the S. Mitchell Mack Hospice House in Mooresville, simply contact the HPCIC office nearest you and ask to speak with our intake specialist.
The intake specialist can provide more information about hospice and palliative care and help you determine whether these options might be the right choice. Hospice & Palliative Care of Iredell County will then work with the patient's doctor for certification into our program.
