Advance care planning is more than just a document, it is a process of planning and conversation.
Simply by having a conversation you can clearly express your wishes to your family and can avoid confusion, conflict, and guilt in a situation that is already very stressful. With a plan in place, you can focus on the things that matter.
To put your wishes in writing, completing Advance Healthcare Directives is very important. The forms are much easier than most anticipate, and they do not cost anything to complete, other than the need for notary services.
