Make a Referral
While it takes a physician’s order to admit a patient to hospice, anyone can make a referral, including friends or family. Hospice & Palliative Care of Iredell County will then work with the patient’s doctor for certification into our program.
We welcome your call. After talking with you, we will then contact the patient and family, and have a conversation with them about hospice care and whether it might be right for them. We also have a wonderful 8-minute video that covers many questions about being a patient with Hospice & Palliative Care of Iredell County. The video provides an excellent introduction to Hospice & Palliative of Iredell County services and is available free of charge to anyone considering hospice care.

To begin your referral of someone for hospice care,
whether for home care, care in a facility or at
the Gordon Hospice
House, simply call our Statesville office at 704-873-4719 or the Mooresville office at 704-663-0051 and ask for the Intake Specialist. The Intake Specialist will obtain information about the individual and will then contact his/her physician to see if the patient is appropriate for hospice care.
Hospice care can be a tremendous help for patients and their caregivers in the last six months of the patient’s life. Many people think hospice care is only for cancer patients. This is a misconception that has kept many from receiving care that could have greatly reduced their pain and suffering at the end of life.
The reality is that only 46% of patients at Hospice & Palliative Care of Iredell County have cancer. Other common reasons for admission include Alzheimer’s Disease, Emphysema, Heart Disease, Multiple Sclerosis, Lou Gehrig’s Disease, Parkinson’s, Liver Disease, HIV, Renal (Kidney) Disease, and more.
Remember, Hospice & Palliative Care of Iredell County can treat patients living in a Skilled Nursing Home or an Assisted Living facility.
