    HOSPICE PHYSICIAN

    A Full Time Physician is needed for the role of Assistant Medical Director with the responsibility to support the Medical Director in the medical direction of the hospice and palliative care program assuring quality care for patients and families.  This position will perform both Hospice patient visits as well as palliative care consultations in homes and facilities as well as our Hospice Unit under the direction of the Hospice Medical Director.

    Preferred candidates will have prior experience working directly with patients and families facing chronic illness and end of life issues, including pain and symptom management background and hospice interdisciplinary team experience.  The ideal candidate will also be Board Certified in hospice and palliative medicine and possess strong communication, leadership, and management skills. 

    Competitive salary and benefit package available.  Two years as a physician with experience in family practice ideal, experience in hospice and palliative care preferred.  Sign on bonus available for the full time role but dependent on experience

    HUMAN SERVICES MANAGER

    A full time (40 hours per week) Human Services Manager position is available. 

    Hours are 8:00am – 5:00pm Monday-Friday. 

    Master’s degree in Social Work is REQUIRED.  LCSW REQUIRED with past LCSW licensure supervision preferred.  Candidate should be an independent, self-starter with strong interpersonal skills. Past human services management experience preferred.  

    Will supervise all social work, chaplain and bereavement staff and coordinate all functions of social work, chaplain and bereavement care provided to hospice patients. 

    Primary responsibilities to include management of the social work team, psychosocial assessments, counseling, and development and implementation of the plan of care related to psychosocial needs.

    This position will also be responsible for the supervision of social work staff and Chaplain Team Lead and will include new patient assignments, annual performance evaluations and corrective actions as needed. Duties will also include selection, hiring, orientation, and training of social workers, chaplains, and bereavement staff with help from the Chaplain Team Lead.

    This position will also oversee social work, chaplain, and bereavement compliance with state, federal, certification and accreditation standards. Oversee statistics gathering and reporting for these teams.  Will also ensure LCSW supervision for HPCIC social workers seeking licensure.

    The Human Services Manager will ensure representation of these teams in IDG meetings, be an active member of the Administrator On-call Team, and a contributing member of the Leadership Team as a representative of the social work, chaplain, and bereavement departments.  Will also champion the organization’s We Honor Veteran’s program.

    Inpatient Unit Manager

    A full time (40 hours per week) Inpatient Unit Manager is a registered nurse responsible for the administrative oversight of the Inpatient Unit in Mooresville, NC (opening spring 2020.)   Should be familiar with state, local, and federal regulatory guidelines, pharmacy regulations, and the concept and philosophy of hospice care.  Responsible for the supervision, planning and implementation of clinical services for the hospice inpatient unit, supervises the interdisciplinary team members, ancillary staff and provides outreach to the community.  Inpatient facility unit manager experience is ideal, and candidate should possess excellent supervisory and organizational skills. Applicant will have 24/7 accountability for the inpatient unit.

    BSN preferred with experience in hospital settings (med-surg, oncology, ICU etc.)  hospice inpatient care and/or hospice homecare.  1 year managerial, supervisory, or administrative experience required and must have or plan to be Certified Hospice and Palliative Registered Nurse (CHPN) within 2 years.

    Should be familiar with state, local, and federal regulatory guidelines, pharmacy regulations, and the concept and philosophy of hospice care. 

    Travel will be required between Mooresville and Statesville.

    Knowledge of Hospice Conditions of Participation (COPs) required.

    Ability to execute an effective, supportive, sensitive approach in response to the needs of patients, families, and staff.

    INSURANCE VERIFICATION/BILLER

     A 50% part-time (20 hours per week) Insurance Verification/Billing position is open with our Pathways Palliative Care Team of Rowan County. 

    As a Pre-Authorization and Verification Specialist you will oversee insurance verification for all palliative care patients, obtain authorizations when needed including physician referrals and special authorizations, and maintain accurate patient information in EMR data management systems. This candidate will also be responsible for completing prior authorizations for medications and DME.

    Candidate must also have excellent organization and communication skills with a working knowledge of medical terminology.

    High school diploma or equivalent required.
